  • You must file a petition to ask the court to establish paternity (who is the legal father), custody, visitation and/or support of a minor child.
  • You may purchase the forms at any Clerk & Comptroller Self Service Center or call (561) 355-7048
  • Scientific testing may be necessary to establish paternity (Florida Statutes 742.12)
