E-Mail UsIf you are filing a Petition For Adoption, please note:
- All records of Adoption are confidential and exempt from the provisions of Section 119.07 (1) Florida Statutes, except as provided in Section 63.162 F.S.
- Subject to inspection only upon Order of Court, the Clerk & Comptroller's office cannot furnish information pertaining to Adoptions to customers. This includes the Attorney of Record and/or the petitioners.
- Pursuant to Section 63.162 F.S., a Court Order is required allowing access to the Adoption file.
- Along with the filing of the Petition for Adoption, you must complete parts A and B of the Certified Statement of Final Judgement of Adoption (Department of Health, Vital Statistics) and file it with the clerk. After the Final Judgement is entered, you must mail a copy of the above along with $20 (unless a check is given to the clerk with the form).
You must attach a check for $20.00 payable to Vital Statistics to the Certified Statement. The Clerk & Comptroller will forward both to the Department of Children & Family Services, P.O. Box 210, Jacksonville, FL 32231-0042. If you do not have this form, you may ask the Clerk & Comptroller for the form or contact the above address.
